What This Is

The Case for an Independent Audit

Since 2003, Iowa’s child welfare system has been reviewed across multiple federal Child and Family Services Review cycles. The Iowa HHS Audit Request organizes public records, federal review findings, open-records correspondence, and source-backed analysis into a public-facing transparency structure. The formal Petition to Audit has not yet been submitted; this hub currently provides a public-safe current-state packet and a verified index of the audit framework.

The public audit framework uses two linked classification systems: Risk Domains A–H and Deficiency Categories F1–F13. Domains organize the source record by system area; deficiency categories identify recurring operational failure types. A dedicated framework page explains the classification system, severity scale, and source-control limits.
