A petition for an independent audit of Iowa’s child-welfare oversight, corrective-action follow-through, and records-integrity standards.
This is the public record for a citizen-initiated Petition to Audit the Iowa Department of Health and Human Services. It documents the petition’s development, materials delivered for preliminary review, and any verified acknowledgments, responses, or follow-up from public officials and oversight bodies.
On August 25, 2026, Ashley Marie Meredith delivered a 62-page Current-State Auditor Review Packet to Iowa Auditor of State Rob Sand during an in-person exchange in Independence, Iowa. The packet presents the stable, review-ready petition core and selected supporting materials as they existed on the date of the handoff.
That handoff was an important step, but it was not the formal petition submission and does not represent acceptance, endorsement, or the opening of an audit. Readers can review the packet below or read Ashley’s account of getting the evidence record into the right hands.
The formal petition remains in final preparation while the remaining sections, attachments, companion reports, and public-source links undergo final verification. This page will be updated as additional materials are finalized and any verified response or follow-up occurs.
This 62-page packet was delivered to Auditor of State Rob Sand for informal, preliminary review. It contains the stable petition core, source architecture, historical context, and analytical standards available on the date of the handoff. It is not the formal petition submission.
The complete petition and supporting record remain in controlled finalization. Current work includes section reconciliation, attachment completion, companion-report integration, public-source verification, and packet-wide quality review. Formal submission has not yet occurred.
Follow the public-record requests supporting this work, including agency correspondence, productions, status changes, response intervals, and unresolved records-access questions.
Follow materials and notices sent to public officials and oversight bodies, together with any verified acknowledgments, responses, or follow-up. The log distinguishes outgoing outreach from documented responses and does not treat silence as a substantive agency position.
